What do the most serious Muskogee County citations involve?

Eight citations in the county reached the immediate jeopardy or actual harm level over the last three years, and six of them fell under the same federal requirement. That rule says a facility must keep its environment free of accident hazards and supervise residents closely enough to prevent accidents, and it is the requirement that applies to most falls. Our guide to nursing home fall lawsuits in Oklahoma explains how those cases work.

Of the other two, one was an actual harm citation for pressure ulcer care, issued to Broadway Care & Rehab Center in March 2025. Pressure ulcers are better known as bedsores, and our page on nursing home bedsore injuries covers what families should know. The other was an immediate jeopardy citation under the federal requirement to protect residents from abuse, issued to Pleasant Valley Health Care Center in August 2024.

Across all 157 citations, quality of life and care was the most common category with 33, followed closely by resident assessment and care planning with 32. Fifty-four of the citations came from complaint inspections, which happen when someone reports a concern to the state and inspectors come out to look into it.

Who operates the nursing homes in Muskogee County?

CMS ownership data lists three facilities under the same chain, MGM Healthcare: Broadway Care & Rehab Center, Eastgate Village Care & Rehab Center, and Fort Gibson Care & Rehab Center. The Springs Skilled Nursing and Therapy is listed under Bridges Health, and the remaining homes are not listed as part of a chain.

When a resident, family member, or staff member reports a concern, the Oklahoma State Department of Health can send inspectors to the facility to look into it. If a facility was cited, does my family member have a case?

Not necessarily. A citation is what inspectors found at a facility on a particular date. Whether your family member was harmed, and whether anyone is legally responsible, depends on their own medical records, the timing, and what happened to them specifically.
